A chain of custody must be included with your samples:
Follow these steps to submit samples:
1. Complete the Homeowner Account Form and print out a copy to include with your samples.
2. Collect your samples and complete the Chain of Custody (COC) to correspond with your samples.
3. Ship of bring your samples to the laboratory. Please note that payment is due before results will be released and the following documents must be provided with samples:
– Homeowner Account form
– Chain of Custody
– Payment or completed credit card authorization form.
